Virginia Beach Public Utilities will mail customers who have testable backflow prevention devices a notification letter and blank test form prior to their annual testing due date. Customers can also download a blank backflow prevention device test form here.

It is the customer's responsibility to ensure the testing is performed annually by a certified backflow prevention device worker and to submit the completed test form demonstrating a passed inspection to Virginia Beach Public Utilities. Customers should keep a copy of their completed test form for their records.
